Appointments

Patients are always welcome into our office at any time. In order to make an appointment, patients should contact the office, preferable by phone. Appointments are going to be scheduled in the best and faster way possible depending on the patient’s urgency, and also the office availability. Office makes those appointments to create an orderly distribution of patients throughout the day; but patients will be seen in the timing way they are arriving into the office. Sicker patients, most of the time, are allowed to get in first.

New Patients

For those coming for the first time, Office recommends to be contacted by phone. Here is a list of things you should have to make a new appointment:

Personal List

  • Name of the patient
  • Date of birth
  • Home address
  • At least one phone number
  • Email

Insurance Information List

Call or log into the Patient Portal and select the date and time for the visit. Patient or guardian must provide the followings documents:

  • Name of the insurance
  • Insurance Policy number
  • Identify if the Policy is: Medicaid
  • Healthy Kid, or Private

Existing Patients

In order to schedule an appointment, they should call the office( always recommended); or just to log into the patient’ portal and select the date and time for the visit. 

For actual visits, patient a guardian has to be able to provide of the followings documents:

Insurance card
Picture Identification document or driver license.
Any medical records from the previous doctor or Hospital (for newborn)
